Premature Baby of about 24 Weeks | Doctor Xydi Gestational Age and Survival 24 weeks = about 6 months of pregnancy. Survival is possible, but outcomes depend on neonatal intensive care availability. Survival rates: In high-resource settings: ~50–60% In low-resource settings: lower due to limited NICU support. Boys tend to have slightly poorer survival than girls at this age. Physical Characteristics Weight: usually 500–600 grams. Length: around 11–12 inches (28–30 cm). Very thin, translucent skin, reddish in color. No fat layer; fragile body. Eyelids may still be fused or just opening. Organ Maturity Lungs: Very immature, surfactant deficiency → needs mechanical ventilation and surfactant therapy. High risk of bronchopulmonary dysplasia (BPD). Brain: Very high risk of intraventricular hemorrhage (IVH), periventricular leukomalacia, and later neurodevelopmental delays. Heart: May have patent ductus arteriosus (PDA). GI tract: Very immature, high risk of necrotizing enterocolitis (NEC). Eyes: Retinopathy of prematurity (ROP) risk due to abnormal retinal vascular growth. Immune system: Weak → prone to sepsis. Skin & temperature regulation: Cannot maintain temperature, needs incubator. NICU Care Needed Respiratory support (ventilator, CPAP, oxygen). Surfactant therapy. IV fluids and parenteral nutrition initially, then gradual enteral feeds via tube. Thermal regulation (incubator). Infection prevention (strict asepsis). Monitoring for complications: IVH, NEC, ROP, PDA. Long-term Outcomes High risk of chronic lung disease, vision problems (ROP), hearing loss, cerebral palsy, and developmental delays. With excellent care, many children can survive and lead meaningful lives, but often with some degree of disability.
