Have a boss soul in your down dpad equipment slot
Have a shield equipped
Have an ashen estus flash (preferred) in your toolbelt
PS4 Controls - Press L2, wait roughly a half second, then press Square, Options, Dpad Down and X before the animation ends, if you see the equipment box pop up and then ask if you want to use the souls, you've done it correctly.
Xbox Controls - LT, wait a half second X, Menu (pause) Dpad Down, A
PC - Use a controller as I do not know what the pc controls are
